Artificial intelligence adoption in healthcare has accelerated faster than governance maturity. Clinicians, administrators, and operational teams are increasingly integrating AI tools into daily workflows for documentation, research, triage support, and administrative automation. However, a parallel phenomenon has emerged as a material enterprise risk. This phenomenon is shadow AI.  Shadow AI refers to the use of AI tools without formal approval, oversight, or security validation by the organization’s IT or cybersecurity function.

Recent surveys indicate the scope is already substantial. More than 40% of healthcare professionals report encountering unauthorized AI tools in their organizations, while nearly one in five admit to personally using them. The intent behind these behaviors are rarely malicious.  Most users cite productivity improvements or a lack of approved alternatives as rational for using unsanctioned AI. Yet the risk profile is significant, especially in environments handling PHI.

Shadow AI vs. Shadow IT

Healthcare executives have long contended with departments or employees using technology systems, software applications, or cloud services without formal approval, security review, or oversight from the organizational IT and cybersecurity departments.  This is known as shadow IT. Historically, this has included unsanctioned file-sharing platforms, collaboration tools, mobile apps, or personal cloud storage introduced by employees seeking efficiency or convenience outside established governance processes. This potentially exposes organizational data, protected information, and trade secrets when systems are breached, misused, or transferred to unauthorized parties.

Shadow AI introduces additional complexity beyond shadow IT.  Shadow AI systems actively process, retain, and potentially learn from submitted data rather than simply storing or transmitting it. Once sensitive information is entered into an external AI platform, organizational control over that data may be diminished or lost entirely, particularly when retention policies, training practices, or vendor data flows are not transparent.

This distinction is critical. Unlike traditional shadow IT tools, AI platforms may incorporate prompts and uploaded content into contextual memory, analytics pipelines, or model improvement processes. As a result, exposure pathways extend beyond direct storage risk to include downstream disclosure through generated outputs, vendor infrastructure, and third-party integrations embedded within AI ecosystems.

The risk is tangible and growing. Data policy violations associated with generative AI tools have increased sharply, with organizations reporting recurring incidents involving sensitive data uploads into unmanaged AI platforms. A substantial portion of these events involves regulated personal or healthcare information, underscoring the potential compliance, privacy, and reputational consequences for healthcare organizations.

Shadow AI Exposure

Several recent developments underscore the urgency of executive attention. Industry surveys from Wolters Kluwer and the Coalition for Health AI (CHAI), published in early 2026, show that shadow AI is present in roughly 40% of hospitals, with 57% of healthcare professionals encountering or using unauthorized tools. These findings demonstrate that shadow AI is not an edge case but an operational reality across the sector.

Healthcare provider organizations have initiated internal privacy reviews after identifying clinicians using public generative AI tools to draft clinical documentation and summarize encounters outside approved platforms. Security analyses describe hospitals investigating these behaviors due to risks of protected health information exposure, lack of Business Associate Agreements, and visibility gaps across clinical workflows. These investigations—often triggered during compliance audits rather than breach disclosures—demonstrate that shadow AI exposure is emerging as an operational governance issue originating within routine clinical activity rather than traditional IT environments. This is an important distinction because it demonstrates how pervasive and diverse the use is.

The issue is also present in emerging clinical AI tools, such as automated medical scribes, which have raised privacy concerns following audits identifying potential exposure of protected health information through third-party analytics and local storage practices. These findings reinforce that even specialized healthcare AI applications may introduce unanticipated data flows.

Data Leakage Pathways

The most immediate pathway to shadow AI exposure is prompt-based data leakage. Clinicians or administrative staff may input patient identifiers, clinical summaries, or operational details into public AI tools without understanding storage or retention practices. Entering protected health information into AI platforms without a Business Associate Agreement can constitute a direct HIPAA violation.

Another shadow AI exposure method involves integration risk. AI tools frequently rely on APIs, plugins, or cloud services that operate outside internal governance frameworks, creating blind spots for data transmission and storage.

Additional threats from Shadow AI include output leakage and personal AI usage for organizational data. Even sanctioned AI systems may inadvertently reproduce sensitive data in responses if trained on internal datasets or session context. However, a greater threat exists when employees use personal AI accounts for workplace content.  I call this Bring Your Own AI (BYOAI), and it is rarely sanctioned, yet highly pervasive.  BYOAI remains widely used in professional contexts, bypassing enterprise monitoring and data loss prevention controls.

Clinical and Operational Risks

Healthcare executives should view shadow AI through three risk lenses: compliance, clinical safety, and enterprise resilience. Compliance exposure is immediate. Unauthorized disclosure of PHI can trigger breach notification obligations, regulatory investigations, and contractual violations. Beyond fines, such events can materially impact reimbursement, accreditation, and payer relationships.

Clinical safety risk is more subtle but equally important. Survey respondents consistently identify patient safety as the top concern associated with shadow AI adoption. Inaccurate outputs, hallucinated clinical information, or altered documentation workflows may influence decision-making in ways that are difficult to detect.

Lastly, operational resilience risk emerges when AI tools influence documentation, scheduling, or revenue cycle workflows without visibility into reliability or availability. Dependency on unmanaged tools can introduce hidden single points of failure.

Risk Mitigation

Shadow AI risk cannot be mitigated solely through prohibition. Governance is only one step in the process.  Effective strategies focus on visibility, governance, and enablement. Visibility is a process of discovery. Organizations must develop mechanisms to identify AI usage across endpoints, networks, and SaaS platforms. This includes monitoring prompt-based data flows and unmanaged accounts.

Once shadow AI use is identified, healthcare organizations should develop clear AI policies.  Such policies should define acceptable AI use, data classification requirements, and approval processes. Employees should then be made aware of the policies and acceptable practices. Training should be reinforced through regular touchpoints to remind clinicians and staff of the organization’s AI policy and goals. 

The goal is not to ban AI. AI is here to stay, but it must be managed appropriately with safe enablement. Providing approved AI tools with security validation reduces incentives for shadow adoption while enabling productivity gains.

Controls and risks must extend beyond organizational boundaries to vendors and partners. The healthcare supply chain is vast and complex. Ensure that vendor systems interfacing with healthcare systems or data are properly vetted. This vetting includes an evaluation of data retention practices, training policies, and contractual safeguards under business associate agreements.

The Strategic Outlook

Shadow AI represents a transitional risk associated with rapid technological innovation. However, healthcare’s regulatory environment and patient safety responsibilities amplify its consequences compared with other industries.

The organizations that navigate this challenge effectively will shift from reactive control models to proactive AI governance frameworks that integrate cybersecurity, privacy, compliance, clinical leadership, and operational stakeholders.

Healthcare executives should view shadow AI not as a temporary anomaly but as a signal of broader transformation in how clinical and administrative workflows interact with intelligent systems.

AI offers powerful benefits to healthcare, but it poses explosive data privacy risks if healthcare executives do not guide its adoption and use.  This allows innovation to occur within a secure, transparent, and accountable framework. Organizations that prioritize visibility, governance, and safe enablement will reduce exposure while preserving the operational benefits that drive AI adoption.
